Should I be concerned about this:

That's DHCP traffic, what kind of network / internet connection is it connected to?

Personally I would not worry about it too much, it looks like a false positive / network spam, I don't think a DHCP packet would be malicious.

Its a BT VDSL connection on a residential premises with dynamic IP. I have checked and my external IP has changed on a regular basis as you would expect. So not sure if something internal is trying to phone home and this is a reply attempt?

Most likely network spam like you say.
